The PDS and care co-ordination improvement programme launched in April 2025. In 2025 we worked alongside 8 teams in cohort 1 and 7 teams in cohort 2.
This is the second year of the programme. In 2026 we will be working alongside 5 teams who make up cohort 3.

Programme support
The support we will be giving to these teams includes:
- improving access to, and the quality and experience of PDS
- improving care co-ordination, for those with more complex needs
- improving specific areas of practice such as personal planning
Sharing learning
We will capture evidence and learning from improvement activity. We plan to share this with our networks to support wider spread. We will do this by:
- hosting a small number of webinars
- publishing progress updates
